Appointment Scheduling & Cancellation Policy
To schedule an appointment please call our office during business hours 915-545-1101 or toll free 1-866-584-8423. In fairness to all of our patients, we are eager to have all scheduled appointments kept. Should you need to cancel your appointment please do so 24 hours prior to the appointment. Without such notice, a cancellation fee of $50 will apply.
Insurance
We accept Medicare and most commercial insurance plans. Patients without insurance are welcome to discuss payment plans. We accept all major credit cards.
New Patient Forms
You can spend less time on paperwork before your first appointment by completing the New Patient Form ahead of time.
Just print and complete the Adobe PDF version Patient Registration Form, and bring it with you to your appointment.
